PA Evaluation Families:

Dear Friends,

2009 Forms Now Available!
All needed forms for 2009 evaluations are now uploaded to this page.
Please review them below and be sure to send a Pre-Evaluation Form
with your portfolio, as well as a Summary Form for each student.
There are directions with each summary form to make it a simple
task to provide the needed information. Please do not use any
forms from previous years. The other two forms are optional 
and are provided for your convenience.

Note that the first postmark deadline date has been extended
to 2/25 to give you some extra days to prepare and mail your
portfolios. All other postmark deadlines remain the same
as indicated in the original Evaluation Notice.

New Families:
Any new family interested in learning more about my evaluation
services should contact me by email with their questions and include
the following information:

--Parent's name
--School district and county
--Grades and/or ages of school-age children
--Number of years homeschooling
--Who referred you to me

Email  is definitely the best way to reach me this time of year.
I will reply to you as soon as possible, but please give me up
to one to two weeks. Call if you do not have access to email.

I look forward to getting acquainted and supporting you in your
homeschool endeavor!

Here are the 2009 Evaluation Forms:

 

Pre-Evaluation Forms: Each family must send a pre-eval form with their
portfolio. Use the form for 'old' families or 'new' families as indicated below.

 

--Pre-Evaluation Form: For Old Families Only

Use this form only if we worked together during the 2007-2008 school year.

--Pre-Evaluation Form: For New Families Only
Use this form if we did NOT work together during the 2007-2008 school year,
even if we worked together in prior years.

Summary Forms: All portfolios must include a Summary Form for each student. Directions are included for your use.

Optional Forms:

--PA Portfolio Summary of Electives:This optional for can be included in your portfolio for subjects thatdo not produce many samples of the student 's work, such as music, physical education, health, safety, etc.

--Log: This optional format for the log allows you to check what subjects were done each day without giving precise details of each activity. You will need to maintain a running list of materialsused in conjunction with this 180-day subject check-off listin order to have a complete log, since the law requires the log to identify by title the reading materials used.

Erie Diploma Transcript Template: Due to me by 10/10/08 if you are getting an
Erie diploma. Please send it to me as an email attachment by 10/10/08.
